This form provides you with information
that may affect your decision as to whether or not to participate in this
research study.
Purpose of the Study
You are being asked to
participate in a study on mental health professional and trainee attitudes
toward working with clients who are working on end-of-life issues.
What will you be asked to do?
If you agree to participate
in this study, you will be asked to complete a survey online. The survey will
take about 15 minutes.
What are the risks involved in this study?
There are no
foreseeable risks to participating in this study.
What are the possible benefits of this study?
You will receive no direct
benefit from participating in this study; however, you will be helping to
further research in this area.
Do you have to participate?
No, your participation is
voluntary. You may decide not to participate at all or, if you start the study,
you may withdraw at any time. Withdrawal
or refusing to participate will not affect your relationship with The
University of Texas at Austin (University) in any way.

Will there be any compensation?
At the end of the survey, you
will be redirected to a new survey where you may enter your email to be entered
to win one of 5 $25 gift cards. Your survey responses and email will not be
linked.
How will your privacy and confidentiality
be protected if you participate in this research study?
No identifying
information will be collected and there is no way to link your response to your
identity.
