To request a forensic evaluation, please complete and submit this form at least 6 weeks before the affidavit is due. Prior to the evaluation, you may be asked to provide a copy of your client’s affidavit, medical records, photographs, and any other pertinent information.

Our clinic currently has the capacity to conduct psychological, not physical, evaluations via video conferencing. However, we can accommodate physical exams in Attorney offices, or other non-clinical spaces provided by the referring attorney. We can provide a clinical space for Ob/Gyn exams on a case-by-case basis. 

We aim to meet the immense need for these evaluations by preparing students for careers serving asylum seekers. Please note that a trained medical student may assist in documenting your client's evaluation. We will honor your client's preference in gender when selecting all participants.
